Prevalence of Acute Hepatitis Among Patients with Dengue Fever at a Tertiary Care Teaching Institute

Abstract
Background Dengue fever is a serious virus spread by mosquitoes that is common in tropical and subtropical areas and is frequently linked to high rates of morbidity and fatality. While classical dengue presents as a self-limiting febrile illness, atypical manifestations such as hepatic involvement are increasingly recognized. Dengue-related liver dysfunction can range from modest transaminase increase to severe hepatitis and liver failure. Early identification of hepatic involvement is crucial for improving clinical outcomes and reducing mortality. Methods This hospital-based analytical observational study was conducted over a period of two years (2021–2023) in tertiary care centers in Navi Mumbai. A total of 100 adult patients (>18 years) diagnosed with dengue fever were enrolled using consecutive non-probability sampling. Patients with other causes of hepatitis were excluded. A detailed clinical history, examination, and laboratory investigations, including liver function tests, were recorded. Statistical analysis was performed using SPSS software, and associations were assessed using the Chi-square, Fisher’s exact test, and Student’s t-test, with p<0.05 considered significant. Results The mean age of patients was 30.12 years, with a male predominance (56%). Common clinical symptoms included abdominal pain (49%) and vomiting (39%). Hepatic involvement was evident through elevated liver enzymes, with 71% showing raised SGOT and 50% showing raised SGPT levels. Mean SGOT and SGPT values were 80.4 IU/L and 58.38 IU/L, respectively. Elevated serum bilirubin was observed in 11% of patients. Ultrasound findings revealed gall bladder wall edema (40%), hepatomegaly (28%), and ascites (16%). A significant association was found between elevated liver enzymes and thrombocytopenia, prolonged fever, and bleeding manifestations such as malena. The prevalence of hepatitis among dengue patients was found to be 28.3%. Conclusion Hepatic involvement is a common and clinically significant manifestation in dengue fever. Elevated transaminases, especially SGOT, are key indicators of liver injury and disease severity. Early detection and monitoring of liver function can aid in timely intervention and reduce complications. Ultrasound imaging complements clinical evaluation in detecting hepatic changes. Awareness of hepatic manifestations in dengue is essential for better management and improved patient outcomes.
