In-Hospital Outcome among Admitted COVID-19 Patients with and without Chronic Pulmonary Diseases

Abstract
Background: Numerous studies have shown that those who have comorbidities and have SARS-CoV-2 may have a
worse prognosis (5,6). Risk factors for COVID-19 patients include conditions including cancer, heart disease,
diabetes, high blood pressure, cerebrovascular illness, and kidney disease
Aim: This study aimed to improve outcome of COVID-19 patient with chronic pulmonary disease.
Patients and methods: The study was conducted as cross sectional study. The study included 210 patients
diagnosed to have COVID-19 and admitted to isolation department of Suez Canal University Hospital for 3 years.

Results: In the current study, associated comorbidities in both groups were studied, showing higher significant
difference in smoking prevalence among CPD group. Moreover, they showed higher prevalence of HTN, chronic
kidney disease (CKD) and ischemic heart disease (IHD). In the current study, COPD patients showed significant lower
SPO2 than asthmatic and non-CPD groups, with no difference when compared with ILD group. They showed
significant difference in inflammatory markers as ESR & CRP on admission when compared with non-CRD group.
They showed the highest levels of ESR, and lower levels of CRP, s.ferritin and D-Dimer when compared with ILD
group. In regard to in hospital morbidity and required intervention, the COPD group showed no significant
difference in NIMV when compared with other groups. They showed lower incidence of ICU admission, ICU stay,
LTOT and mortality rate and IMV when compared to ILD group. When comes to hospital stay, they showed the
longest hospital stay but without significant difference with ILD group.
Conclusion: This study illuminates the substantial burden of chronic respiratory diseases on COVID-19 patients’
outcomes, demonstrating a clear association with increased risks of hospitalization and mortality. These findings
reinforce the critical need for targeted healthcare strategies aimed at managing CRDs & COVID-19 patients more
effectively. By addressing these conditions with greater precision and care, healthcare systems can potentially
reduce the incidence of severe clinical events and improve the quality of life for affected individuals.
