Functional Outcome Of Transforaminal Lumbar Interbody Fusion (TLIF) With Unilateral Pedicle Screw Fixation In Lumbar Degenerative Disc Disease: A Prospective Study.

Abstract
provides biomechanical stability, and therapeutic benefits. Some studies have shown that excessive stiffness due to the procedure may impede fusion. The unilateral pedicle screw technique may decrease thecal sac and nerve root retraction while maintaining contralateral structures. Therefore, aim of study was to analyse the functional clinical outcomes, quality of radiological fusion, and complications in patients with Lumbar degenerative diseases (LDD) who have undergone open transforaminal lumbar interbody fusion (TLIF) approach with unilateral pedicle screw fixation.
Materials and Methods: 20 patients underwent the aforementioned surgical technique for their LDD between June 2019 and August 2020. The results were evaluated based on visual analogue scale (VAS) and Oswestry Disability Index (ODI) were recorded. The radiographic examinations in form of X-ray, computed tomography (CT) or magnetic resonance imaging (MRI) was done preoperatively and 3 months, 6 months and 12 months postoperatively. The paired t-test was used for comparison between the preoperative and postoperative values. P < 0.05 was considered to be statistically significant.
Results: The mean pre-operative VAS score was 7, which dropped to 4.25 at 3 months, 2.2 at 6 months, and 0.8 at 1 year (P<0.00001). The mean ODI score was 49.70 before surgery and decreased sequentially: 34.50 at three months, 21.50 at six months, and 15.40 after one year(P<0.00001). The present study observed 90% grade 1 fusion, 5% grade 2 and 5% grade 3 fusion rates at 1 year postoperatively. Study found 85% participants without complications and 5% each with CSF leak, dural rupture, and urine retention.
Conclusions: Open TLIF using unilateral pedicle screw fixation is a safe, feasible and effective technique in the treatment of LDD and yields functional and radiological outcomes that are comparable to those achieved with bilateral pedicle screw fixation
