Cystic lymphangioma is a rare benign congenital malformation, most commonly found in the cervical region of the neck. Although typically diagnosed in childhood, its presentation in adults is unusual, often leading to diagnostic and management complexities. This study aims to explore the clinical features, diagnostic approaches, and management of cystic lymphangioma in adults. We present a case series of four adult patients diagnosed with cystic lymphangioma, each with distinct presentations and clinical challenges. The cases were evaluated using clinical suspicion, imaging techniques such as ultrasonography, CT scans, and MRI, followed by histopathological examination for confirmation. The study highlights the varied presentations of cystic lymphangioma in adults, which include slow-growing, painless, and sometimes compressive masses. Diagnostic procedures often involve non-invasive imaging techniques, but histopathological evaluation remains the gold standard. The management strategy primarily involves surgical excision, which, in most cases, provides effective results. However, the recurrence rate, particularly if the cystic mass is incompletely excised, ranges from 10-15%. The study emphasizes the importance of early diagnosis and timely intervention to reduce complications and prevent recurrence. Additionally, it discusses the rarity of this condition in adults, where trauma, infection, or neoplasm are suspected etiological factors contributing to its manifestation. In conclusion, cystic lymphangioma in adults, though rare, can be effectively managed with early diagnosis and complete surgical excision. Follow-up care is essential to monitor for recurrence. Given the complexities of diagnosis and treatment, awareness of this condition among clinicians is crucial for improved patient outcomes.
