Comparison of Urine Acetoacetic Acid and Capillary Beta Hydroxybutyrate in the Diagnosis and Management of Diabetic Ketoacidosis: A Prospective Observational Study from a Tertiary Care Hospital

Abstract
Background Diabetic ketoacidosis (DKA) is a life-threatening acute metabolic complication of diabetes mellitus characterized by hyperglycemia, ketosis, and metabolic acidosis. Conventional diagnosis relies on urine ketone testing using nitroprusside reaction, which detects acetoacetate but not β-hydroxybutyrate (β-OHB), the predominant ketone body during DKA. Early diagnosis and monitoring are crucial for reducing morbidity and mortality.
