Comparison of Transcranial Doppler Parameters in Migraine Patients During Ictal and Interictal Phases

Abstract
Background: Migraine is a prevalent neurovascular disorder associated with altered cerebral hemodynamics. Transcranial Doppler (TCD) ultrasonography provides a non-invasive tool to investigate these changes during ictal (attack) and interictal (attack-free) states. Objective: To compare TCD parameters in migraine patients during ictal and interictal phases, and to evaluate differences with healthy controls. Methods: Ninety participants were enrolled: ictal migraine (n=30), interictal migraine (n=30), and healthy controls (n=30). TCD was performed to measure mean flow velocity (MFV), peak systolic velocity (PSV), end-diastolic velocity (EDV), pulsatility index (PI), and resistive index (RI) in the middle cerebral artery (MCA), posterior cerebral artery (PCA), basilar artery (BA), and vertebral artery (VA). Statistical significance was set at p<0.05. Results: Migraine patients exhibited higher PSV and PI in the MCA and PCA compared to controls. During ictal phases, PSV and PI were consistently elevated relative to interictal and control groups. VA showed significantly higher MFV in ictal compared to interictal groups (p=0.04). No significant changes were observed in EDV.
